Membership Application
There are two ways to complete a Membership Application using the Humane Society of Central Illinois website.
HSCI Pets

Paperless / Credit Card Select the membership level you would like to contribute and complete payment through PayPal. After completing payment you will be returned to the Membership Application webpage to complete a very brief form. PayPal accepts Visa, MasterCard, American Express, and Discover.

Paper / Check / US Mail Print the Membership Application form / webpage, complete the form, and mail the completed form along with your check, to the HSCI.
